To apply for your organization's membership with LAHP, please complete the form below.  Afterward, you will have the option of submitting payment for your annual dues online.  You may also choose to pay by check or speak with a LAHP representative about payment.

Registration by Fax or Mail

Complete the printed membership application form and send it to LAHP via one of two methods: 

1.  Fax it to LAHP at (225) 756-3228.  If any amount is due, you will receive an electronic invoice from LAHP and can pay by company or personal check, or credit card.

2.  Mail it to LAHP at 17425 Jefferson Hwy, Suite C, Baton Rouge, LA, 70817, accompanied by payment with a company or personal check.
